Lauren Hammond, MS, CCC-SLP
Speech-Language Pathologist | PhD Candidate | Clinical Care Specialist
I’m a licensed Speech-Language Pathologist with a deep-rooted passion for helping adults live fuller, more connected lives—especially in the face of neurological change. I specialize in adult communication and cognitive disorders, with a clinical and research focus on stroke, brain health, and communication recovery. As a current PhD candidate, my work explores how language, cognition, and life participation intersect—because I believe that care should support not just survival, but a vibrant quality of life.
In addition to my clinical and academic work, I’m deeply committed to giving back. I run community-based support programs in Columbia, South Carolina, aimed at empowering individuals with aphasia and other neurological conditions. Whether it’s organizing life participation groups, building education initiatives, or supporting families through their care journey, my mission is to ensure every person has the tools and support they need to thrive. At the heart of what I do is a love for people, stories, and the power of communication.

My daughter and I created this care team together with one goal in mind, to support families like ours who are navigating the many challenges of aging and care decisions. Before stepping into this role, I spent many years working in real estate. But over time, my path shifted as I became a caregiver and advocate for several of my own family members navigating serious serious health and cognitive challenges.
I’ve helped coordinate transitions into skilled nursing, arranged in-home care when other options fell short, and supported loved ones through memory loss and increasing daily care needs. I know firsthand how overwhelming this journey can feel—because I’ve lived it with the people closest to me.
That personal experience drives everything I do in this role. I understand the emotional weight of making care decisions, and I’m here to help families feel supported, informed, and never alone. Whether I’m coordinating services or offering a listening ear, I bring empathy, organization, and heart to every interaction.
